To register for an NWA training seminar:

	Call NWA Seminar Coordinator Lynn Theis at 503-224-7727
	or
	Print the Registration Form, fill it out, and send it to NWA at the address on the form.

Notes:

	Seminars often fill quickly. We recommend registering by phone to guarantee your place.
	Two weeks prior to class, a confirmation packet will be mailed to you with complete details regarding schedules, accommodations, and other relevant information.

Cost:

	$995 for Quality Analyst seminar
	$700 for Quality Monitor seminar
	Payable in full upon registration
	Transportation and accommodations not included

Payment:

VISA, MasterCard, American Express, Company checks

Cancellation:

Cancellations received at least 21 days prior to the opening date will be subject to a $150 fee before refund. Any cancellation received after that date will result in forfeiture of the entire registration fee, with no refund issued.
